25 | Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Alecia Sifuentes met with Licensee, Sarah Kelly for the purpose of a case management inspection. Licensee requests a decrease in capacity. Licensee is removing the toddler option program and requesting a change of capacity for 12 children ages 0-24 months only. The fire clearance (STD850) was granted and received on 11/16/2023. The program currently operates Monday through Friday from 6:30 a.m. to 6:00 p.m.
INDOOR ACTIVITY SPACE:
There are two infant classrooms. The crib area is located in the larger infant classroom. LPA observed five cribs separated by a four-foot-tall wall partition and seven nap mats. LPA remeasured the large infant classroom and second infant classroom. The total classrooms space contains a total of 449 square feet, which will accommodate Licensee’s requested for 12. There is 1 toilet and 2 sinks for the children. AS observed a changing table within arm's reach of a sink in both classrooms.
Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Exit interview conducted and report was reviewed with the Licensee, Sarah Kelly.
Effective today, 11/27/2023, LPA will approve the change of capacity for 12 infants. |
