On 2/22/24, Licensing Program Analyst, Carla Polanco (LPA) and Licensing Program Manager, Karyn Guerra (LPM) conducted an unannounced field visit to deliver the findings for the above allegations. LPA and LPM arrived at the facility and were met by Licensee Sarah Kelly (D1). LPA disclosed the purpose of the inspection and were granted entrance. LPA and LPM toured the facility, there were 42 children and 9 staff present during today’s inspection. LPA determined through Guardian verification that all required adults were background cleared. Throughout the course of the investigation, LPA reviewed the facility’s files, collected documents pertaining to the allegations, and conducted observations and interviews.
It was alleged that the facility is operating out of ratio. LPA conducted interviews with parents and staff to obtain more information regarding ratio being maintained at the facility. Based on the information obtained through interviews as well as inspections and census counts conducted by LPA, it was revealed that although the facility has had issues maintaining ratios in the past, ratio is presently being maintained and capacity regulations are being followed.

It was alleged that staff are not appropriately addressing the needs of children while in care. Interviews that were conducted with various parents and staff revealed no corroborating statements that behaviors and needs of children are not being properly addressed or communicated with parents.
It was also alleged that staff are not properly being trained. Due to mixed statements received during interviews with staff, LPA was unable to determine if adequate training is being provided to staff.
Although the allegations may have happened or are valid, no disclosures nor evidence support that the incidents described occurred. Therefore, based on the collected information, the findings are UNSUBSTANTIATED as there was not a preponderance of evidence to prove or negate the specific allegations.

An exit interview was conducted with Licensee. A notice of site visit was provided and posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
