On 6/9/2023 at 9:25am, a Required-1 Year inspection was made to the facility by Licensing Program Analysts (LPAs), Laura Chavez and Julie Gifford. This program is operated by Head Start and operates 5:00am - 5:00pm, Monday–Friday. The facility was toured at 9:50am inside and outside and the floor and yard plan submitted by the licensee were verified. The outdoor activity space was cushioned with wood chips and free of hazards. The preschool program operates in Room 3.

There were four teachers supervising seven children, and operating within the licensed capacity and ratio requirements. There are no pools or bodies of water on the premises

Five children's records were reviewed at 10:34pm. Five staff records were reviewed at 1:00pm.

Center Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following deficiency was cited: 1597.16(a)(1) Lead Testing, a review of the Child Care Center Sampling Checklist Form received on 5/3/2023 confirms the testing for lead in the water was completed on 4/29/2023, (see LIC 809D).

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted and report was reviewed with Center Director Andrea Navarro.
