25 | On 5/24/24 Licensing Program Analysts (LPAs) Monica Mathur and Kareeca Sykes conducted an unannounced Case Management inspection at Valle Verde Children's Center and met with Director, Christine Muller. Purpose of today's inspection is to investigate few unusual incidents that occurred in the program recently.
On 5/20/24 during an unannounced inspection, it came to light when Director mentioned to the LPA about an incident that occurred on 5/17/24. A child C1 fell in the playground after being pushed by another child C2. Two staff who were present, actively providing supervision and saw the incident happen. Staff immediately attended to the injured child who complained of arm pain by providing ice pack. Parents were informed immediately, child was picked up and required medical attention. There were no internal injuries and child returned to the program after the weekend. Supervision training was provided to all staff the same day of incident.
Director also mentioned to LPA another unusual incident that occurred later the same day on 5/17/23 in the Multi-purpose room. A child asked teacher S1 to use the bathroom. S1 allowed the child to leave the Multipurpose room without visual supervision through the back door. Director saw the child come out the back door and took child back to the room. Per Director, C1 was alone for no more than 2 seconds. Director and Assistant Director, Kristina Sisco immediately spoke to S1 about lack of supervision and oversight.
20 minutes later another child attempted to leave the room and was caught by staff before leaving the room. Director states S1 was provided supervision training earlier same morning after the injury incident. Since the supervision incidents S1 has left the job. Director states even though they provide ongoing training to staff on supervision requirements, S1 had a history of lack of supervision in the past.
Facility is reminded to stay in compliance with requirements of supervision. 100% visual supervision must be always provided, and no child should be left unattended at any time.
