On 06/09/2026, Licensing Program Analysts (LPAs), Dana Santiago and Vandana Eswara arrived for an unannounced complaint investigation visit to deliver findings for a complaint received. LPAs met with center Director Racheal Mercado. Twenty day care children and 9 staff with director on site were present during this visit.
Complainant alleged that staff do not provide adequate care and supervision resulting in child's injuries.
During the course of the investigation through interviews and record review, it was determined that a child was injured by another child due to lack of supervision. The preponderance of evidence standard has been met, therefore this allegation was found to be SUBSTANTIATED.
Although the allegation is substantiated, the facility is not being issued a citation today as the facility was already issued a Type A citation through a previous visit conducted by another LPA on 04/17/2026.
Exit interview was conducted with facility director Racheal Mercado, and a copy of this report provided.
The Notice of Site Visit form was provided and must be posted for 30 days.

On 06/04/2026 Licensing Program Analysts (LPAs) Dana Santiago and Vandana Eswara conducted a subsequent inspection to deliver the findings to the above complaint allegation. LPAs met with center Director Racheal Mercado. Twenty day care children and 9 staff with director on site were present during this visit.

Complainant statement alleges that the staff do not keep the center free of pest. Further investigation has been conducted. LPAs conducted interviews, record reviews, and a facility observation on multiple visits.
Due to conflicting information gathered, LPAs determined although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore, the allegation is UNSUBSTANTIATED.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Facility Director Racheal Mercado
