On 2/11/2025 at 10:45 AM, Licensing Program Analyst (LPA) A. Gomez arrived announced to conduct 1-Year Annual Required inspection. LPA met with Licensee, Norma Loquellano and explained the purpose of the visit. The facility is temporarily closed due to a fire that occurred in April of 2024.

LPA observed that the facility is still under construction. The facility does not currently have running water, electricity, flooring or walls. The facility is currently gutted and undergoing construction. There are no staff or residents. Facility is projected to be done with construction November 2025. LPA advised Licensee to notify the department prior to reopening/ accepting clients so that the department can do a new inspection.


No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
