25 | On 09/15/2023 at 8:30 AM, Licensing Program Analysts (LPAs) Christina Watts and Ashley Akinleye conducted an unannounced Annual Inspection at Meadowlark Children's Center. LPA met with Director Giovana Merritt and explained the purpose of today's inspection. The facility is located on the campus of Lucille Mauzy School. No children were present during inital inspection as the facility opens at 9:00 AM. Present during inspection were 3 staff including the Director. Director stated there were 33 children enrolled. Director stated this is a part day facility that runs 2 programs. Facility's operating days and hours are Monday - Friday from 9:00 AM - 1:00 PM in 1 Room. All staff present, caring and supervising children had Criminal Record Clearance.
The physical plant was inspected. LPA toured the premises.
Indoor space: 1 classroom, 2 restrooms were inspected. During inspection, children were not present as\t the facility as it opens at 9:00 AM. Disinfectants, cleaning solutions, and other items that are dangerous to the health and safety of children were stored in places inaccessible to them. Cabinets, drawers, and rooms used for storage were locked. and equipment were age appropriate and in good condition, free of sharp, loose, or pointed parts. Restrooms for children were observed to be in safe, sanitary, and functioning condition. Floors were clean and free from tripping hazard. Foods and beverages were stored safely. Food storage area were clean, free of litter, rubbish, and rodents/vermin. Trash cans for solid waste had tight-fitting covers on and were in good repair. LPA observed a working Fire extinguisher, Smoke and Carbon Monoxide Detectors. Log shows that the last Fire Drill was conducted on 08/22/2023. LPA observed 2 guinea pigs in a cage in the facility. Facility does not provide transportation for children, but Director understands that children cannot be left alone, unattended in parked vehicles. Facility’s License, Parents’ Rights Poster PUB 393, Personal Rights, Activity Schedules, and Menus were observed to be posted.
Outdoor Space: Outdoor playground was inspected and observed to be fenced and safe. The play equipment was maintained in good condition and free of hazards. *CON'T ON PAGE 2*
