On 4/16/2026 at 9:30 AM, Licensing Program Analyst (LPA) Kayla Merchant conducted an unannounced Subsequent Complaint Investigation at Angels Montessori Preschool - Concord. LPA met with Director, Maryam Karampour, and explained the purpose of today's investigation. Complainant alleges that staff did not provide adequate supervision resulting in child wandering away from the facility. There were 3 staff and 17 children present during today's visit.

During course of investigation LPA conducted facility inspection, observations, record review, interviews and obtained documents. It was determined that during the children's lunch time, a 5 year old child walked out of the classroom, then out of the front door. That child was found crossing back and forth across a busy road that is right outside the facility by 2 motorist. The child was returned to the facility by the 2 motorist. 2 staff, who were supervising the children during lunch, stated that they initially did not realize the child was missing, but once they did, the motorists were ringing the doorbell to return the child. it was stated that the child had been unsupervised for appoximately 5 to 10 minutes.

Based on the interviews and information obtained throughout the investigation, the preponderance of evidence standard has been met. Therefore, the allegation is SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 is being cited on 9099-D page.

This is a zero tolerance violation. An immediate $500 is assessed today and $100 per day will be assessed until corrected. Subsequent zero tolerance violations are $1000 immediate civil penalty and $100 per day will be assessed until corrected.
A civil penalty of $250 was also assessed and issued during today’s inspection as this is a repeat violation.

LPA Kayla Merchant informed director Maryam Karampour that this report dated 4/16/2026 documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Kayla Merchant informed the director to provide a copy of this licensing report dated 4/16/2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview was conducted and report reviewed with director, Maryam Karampour.
A NOTICE OF SITE VISIT WAS ISSUED AND MUST BE POSTED FOR 30 DAYS.

The director will submit a summary of the supervision training she conducted after the incident occurred and staff will complete a supervision handout that will be submitted along with the training summary to LPA by 4/17/2026.
