On 7/8/2025 at 9:00am Licensing Program Analyst (LPA) Morgan Pringle met Licensee Kumudini Sharma for an Unannounced Annual/Random Inspection. Present during the inspection was the Licensee, Licensee’s adult helper, one (1) infant and six (6) preschool age children. Three (3) additional children arrived later in the morning. Licensee’s husband, co-licensee, Rajesh Sharma, was home as well and left around 9:15am and returned around 10:00am. Licensee lives in the home with her husband. Licensee’s home was toured for a health and safety inspection. The facility operates 8:00am – 5:30pm, Monday – Friday.

ON LIMITS AREA: Family room, dining room (used for napping and play), dining area (next to kitchen), living room, front porch (leads to family room), downstairs half bathroom and backyard
OFF LIMITS AREA: Entire 2nd floor, downstairs den, downstairs bedroom with attached bathroom, kitchen and garage
ISOLATION AREA: Family room, dining room or living room

The facility is a two-story home owned by the Licensees. The inside of the home was observed to be neat, clean with ample age-appropriate materials for the children’s learning and play. All toxins, cleaning products, personal medications, and hazardous materials were observed to be in inaccessible areas. Licensee stated she provides all food for the children. All food that may be brought from the children’s home will be properly labeled and stored.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Licensees use child sized tables and chairs for meal times. All materials used for eating were observed to be well maintained, free from defects and in proper working order. Licensee stated all diapering materials are brought from the children’s home. LPA observed three (3) cribs used for napping that were free of defect and in proper working order. All napping materials are brought from the children’s home and was observed to be clean and free from defects. Licensees do not transport children, there are no firearms, and there is one (1) dog in the home.

LPA observed one (1) fully charged 3A40BC fire extinguisher in the kitchen. There is one (1) working smoke/carbon monoxide detector in the dining room and living room and hallway leading to the backyard. There is one (1) smoke detector in the hallway leading to the off-limit downstairs bedroom. The home is equipped with central heat and air for proper ventilation. The staircase is gated making it, and the 2nd floor, inaccessible to the children in care. The electric fireplace in the family room is locked and does not get hot to the touch making it inaccessible to the children in care. LPA did not observe any bodies of water in or around the home that could be a potential danger to the children in care.

The backyard is fully fenced, clean and well maintained with ample age-appropriate materials for the children. There is a large pavilion with an outdoor kitchen that is locked and a firepit that is filled with grass making them inaccessible to the children. There is a basketball hoop and two small climbing structures as well. There is a shed on the right side of the home that is gated and inaccessible to the children. Licensee’s neighbor on the left side has a swimming pool and LPA verified that the fence between the two homes was free from defects and well maintained. There are no pools in Licensees home.

The facility is operating within its licensed capacity. Licensee’s Health and Safety training have been completed and EMSA approved Pediatric CPR & First Aid has been completed for Rajesh Sharma and expires 5/4/2026. Kumudini is scheduled to renew the training on 8/2/2025. Both Licensee’s Mandated Reporter trainings have expired as of 1/10/2025 (see LIC809D). Licensee and LPA agreed that the training will be complete by 7/11/2025. Fire/disaster drills have been conducted and recorded with the last drill logged 3/11/2025. All required forms are posted and visible for public view on the wall in the living room. LPA verified all adults living and working in the home have obtained a criminal record clearance. LPA obtained the children’s files, and facility files. All files were complete.

Licensee's helper left the home at 12:20pm leaving Kumudini in the home alone with 10 children, taking her out of ratio. LPA informed Licensee about the issue and Licensee stated that her next assistant wouldn't arrive until 2:00pm. Licensee stated that since it is nap time and the children are usually asleep she thought it was alright to have the break in time that the assistant come. Licensee stated that she has had the same schedule for her assistant for many years and no one has said anything. LPA reviewed previous reports and noticed that Licensee was in compliance previous years or an LPA had come when an assistant was present, before or after nap time. LPA informed Licensee that the facility must remain in ratio at all times (see LIC809D).

Deficiencies cited during inspection
  • Both Licensee's Mandated Reporter training are expired
  • Licensee out of ratio from 12:20 to 2pm


Licensee was reminded that California law requires Licensees to report unusual incidents and/or injuries to children in care, to the child's authorized representatives, and to Community Care Licensing Division (CCLD) within 24 hours by phone. Within seven (7) days of the incident, Licensees must submit the Unusual Incident/Injury form (LIC 624B) to CCLD. Licensee was reminded that any structural changes or additions to the home must be reported to CCLD. Children’s Roster must be properly maintained, and fire/disaster drills must be conducted every six (6) months and documented.

EMSA approved Pediatric CPR & First Aid training must be completed every two (2) years. Mandated Reporter Training ("Child Care Providers") is required for all staff and is to be renewed every two (2) years by visiting mandatedreporterca.com/. LPA informed Licensee that all forms can be downloaded at www.ccld.ca.gov.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/resources/child-care-centers/.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, Licensee Kumudini Sharma, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.
