On 11/5/2025, Licensing Program Analysts (LPAs) Dana Santiago and Ashley Hollinger conducted an unannounced Subsequent Complaint Investigation at Lifepoint Preschool. LPAs met with Arlene Mclean and explained the purpose of today’s inspection. During today's inspection, there was 6 infants in care. The finding for the above allegation was delivered during the inspection. Complainant alleges that staff caused bruising to infant. During the course of the investigation LPAs completed a physical plant inspection, reviewed facility records, and conducted interviews. Based on the interviews and information obtained throughout the investigation, the allegation is UNSUBSTANTIATED which means although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. No deficiencies have been cited for this allegation.

Exit interview conducted with Arlene Mclean. Notice of site was given and must be posted for 30 days. Appeal rights were provided.

On 11/5/2025, Licensing Program Analysts (LPAs) Dana Santiago and Ashley Hollinger conducted an unannounced Subsequent Complaint Investigation at Lifepoint Preschool and met with the Arlene Mclean and explained the purpose of investigation. Complainant alleges that the staff co-mingle preschool child with infant program. During course of investigation, LPAs conducted facility inspection, observations, and interviews. Through multiple interviews it was determined that a preschool aged child was in the infant classroom on at least one occurence while a parent was taking another child to the bathroom. It was determined that the facility is not in compliance with the scope of their license which poses a potential risk to the Health, Safety or Personal Rights to children in care.
Based on the interviews and information obtained throughout the investigation, the preponderance of evidence standard has been met. Therefore, the allegation is SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 is being cited on 9099-D page.
Exit interview was conducted with Arlene Mclean. A NOTICE OF SITE VISIT WAS ISSUED AND MUST BE POSTED FOR 30 DAYS. Appeal Rights Provided.
