13 | On November 8, 2022 at 8:20 am Licensing Program Analyst (LPA) Nyeesha Blount and Licensing Program Manager (LPM) Mayla Mendoza conducted an unannounced complaint investigation inspection regarding the above allegation. LPA and LPM entered the facility at and met with Director Ruth Watkins. Present during the inspection were 3 teachers, (4) Preschool, and (5) Toddlers present for a Health and Safety inspection,
During the investigation LPA conducted staff and Parent Interviews. This agency has investigated the complaint alleging staff did not pick up day care child from school in a timely manner. Based on LPA's interviews which were conducted and a record review, provider was not aware of the new pick up location in advance, but text threads between parent and provider indicated provider was given new pick up location.
although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur. Therefore, the allegation is UNSUBSTANTIATED.
Exit interview conducted with Director Watkins, Ruth and appeal rights were provided and discussed.
A notice of site visit was given and must remain posted for 30 days. |
