On 8/11/22 at 10:55am Licensing Program Manager (LPM) Loretta Dyson arrived at the center for an unannounced case management inspection. LPM met with Ruth Watkins, center owner/director. There were 3 additional staff members and 13 children also present. An application was received, to change the portable from outdoor use to indoor use, and increase the capacity of the preschool license. LPM arrived to complete the inspection for the requested change. LPM toured the classrooms, portable, bathrooms and outdoor space with the director.

LPM measured the portable, to add to the indoor square footage for this license, and the measurement is as follows: 183.19 square feet. A fire clearance, dated 8/5/22, was received from the City of Richmond Fire Department.

At 11:50am, LPM confirmed, from the facility roster in Guardian, that one staff member does not have a criminal record clearance in Guardian. According to the director, the staff member began employment on 8/8/22. The director advised that the staff member works at another facility that she believed was a licensed child care facility, but she has not been able to confirm that.

The portable will be licensed for use as an indoor classroom once the following is complete, due to it being a separate building: a functioning smoke detector is installed; a functioning carbon monoxide detector is installed; and a fire extinguisher that is a 2A10BC or higher in size is placed in the portable.

A Type A deficiency is being cited today, so a copy of this report needs to be given to each parent or guardian by the next business day and the signed LIC 9224- Acknowledgement of Receipt of Licensing Reports placed in each child's file. A copy of the report shall be given to the parent/guardian of newly enrolled children for the next 12 months, and the LIC 9224 signed and placed in the child's file as acknowledgment of receipt of the report. A copy of the report with deficiency shall be posted for 30 days, in a place that is visible to the public.

The director agreed that M. Walker will not work at the center until she obtains a criminal record clearance. LPM will confirm that D. Coleman is no longer working at the center by close of business on 8/12/22.

This requirement has not been met as evidenced by LPM's observation of staff member M. Walker caring for children. The director confirmed that she began working at the center on 8/8/22. LPM checked and found that M. Walker does not have a criminal record clearance. This poses an immediate risk to the health and safety of children in care.

An immediate $400 civil penalty is assessed today and $100 per day will be assessed until corrected. Subsequent violations are $1000 immediate civil penalty and $100 per day will be assessed until corrected.
