On 10/20/21 at 12:30pm, Licensing Program Analysts (LPAs) Melissa Guirit and Melissa Domantay arrived at the facility for an announced prelicensing inspection. LPAs met with applicant and Director Ruth Watkins. Also present were applicant's parents and brother. The applicant submitted an application for a school age component license at this facility. Also on site will be a preschool age component. The facility will operate Monday-Friday 6:00am-6:30pm in half of the big room that is physically separated by a wall from the preschool/toddler component. Staff bathroom inside building is OFF LIMITS to children. Parent board is in the entrance in the hallway. LPAs toured all areas to be used by children, and the measurements
are as follows:

INDOORS: 759.72 square feet= 21 children
OUTDOORS: 2417.12 square feet= 32 children

A fire clearance, with a capacity of 25 school-age was received from City of Richmond Fire Department.
LPAs observed that all classrooms have sufficient lighting, heating and ventilation for the safety and
comfort of children and staff. The floors and surfaces were clean and in good condition. LPAs observed an
ample supply of age appropriate activities, equipment, and furniture that is in good condition. The climbing
structure in the play yard has rubber mulch as sufficient cushioning underneath to absorb a fall. A outdoor portable will also be utilized by the school age component when outdoors. The play structure age range is from three years old to twelve years old indicated on the manufacturer label. The outdoor play area is fully fenced and has an ample supply of toys and activities. There is also another fully fenced play area in the front that can be utilized by the children with 100% supervision. LPAs did not observe any defects or dangerous conditions in the outdoor play areas.

There is 2 toilets and 3 sinks available for children, and they are in sanitary and operational condition. There are two separate restrooms that will be utilized. There is a separate bathroom for staff. The staff office will be used for isolation of sick children. The facility is equipped with 3A40BC fire extinguisher, centralized smoke and carbon monoxide detectors, telephone, and first aid supplies. LPAs did not observe any hazardous items, toxins, bodies of water or medication accessible to children today. The facility will provide breakfast, AM and PM snacks, and lunch for children. Children will have individual water bottles, which will be refilled as needed, so there will be water available both indoors and outdoors. The COVID-19 Assessment will be provided to applicant and will be submitted to LPA via email.

If This facility plans to provide Individual Medical Services – IMS. A Plan of Operation that includes IMS must be submitted to the Department. Ruth Watkins is a fully-qualified school age director with a site supervisor permit and has completed 16 hours of health, and safety training, lead poisoning prevention training, and pediatric CPR and First Aid. Mandated reporter and appeal rights, civil penalties, unusual incident reporting and fingerprint requirements were discussed today. Licensee is also being informed of the web address (www.ccld.ca.gov) for downloading child care forms, and the director is encouraged to email ChildCareAdvocatesprogram@dss.ca.gov to be included in the Child Care Quarterly Updates distribution list. The director is also reminded that mandated reporter training is required for all staff and is to be renewed every 2 years at www.mandatedreporterca.com. Zero Tolerance policies were explained. The center was found to be clean, safe, sanitary, and in good repair.

A license for 21 school age children will be effective today 10/20/21. An exit interview was conducted, and a copy of the report and appeal rights were provided to applicant.
