On 09/17/2026 at 9:37 AM Licensing Program Analyst (LPA) Kareeca "Reeca" Sykes conducted an Announced Prelicensing Inspection at the applicant Juana Valdez home. Upon arrival LPA was greeted by the applicant and was granted entry to the home. There were no daycare children present during the inspection. Living in the home is the applicant, and the applicant's husband. All adults living and working in the home are fingerprint cleared. The home was toured with the applicant to conduct health and safety inspection. Applicant states the hours of operation for day care will be Monday through Friday, 5:30 AM to 6:00 PM.

This is a two story home with four bedrooms, three bathrooms, kitchen, dining area, living room, laundry room, basement, backyard, deck (front of home) and garage. LPA did observe a fireplace in the living room area of the home, which the applicant stated they would only use when children are not present. LPA has confirmed that the fireplace is made inaccessible to children in care by a safety gate. The home is neat and clean with heating and ventilation for safety and comfort of children in care.

ON LIMITS: Living room, dining room, basement (to the left of the living room - down the stairs), bathroom 3 (located in the the basement), and the fully fenced backyard (not including the left side yard).

OFF LIMITS: The entire upstairs (including bedroom 1, bedroom 2, bedroom 3, bathroom 1, and bathroom 2), the kitchen the laundry area, the garage, and left side of backyard. All off limit areas will be inaccessible by closed doors with safety latches, safety gate and visual supervision. The applicant was advised to contact Licensing, so that an inspection can be completed prior to changing any off limits area to on limits.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

ISOLATION AREA: In the living room area, at the table away from other children in care.

The outdoor play area will be in the backyard. Applicant has also stated that, they will also take the children to the park (mountain park) down the street from the home occasionally. There are ample age appropriate toys which are observed to be safe, clean and in good repair. LPA did observe a waterfall pond in the backyard which has been made inaccessible to children in care. LPA did not observe any pools or hot tubs in the home. LPA did not observe any hazardous materials or toxins accessible to children today.

The home has a fully charged 2A10BC fire extinguisher located in the kitchen/dining area, working carbon monoxide and smoke detectors, telephone, and fully stocked first aid kit. Heater vents are located on the floor, which LPA confirmed does not get hot to the touch. Per applicant, there are no firearms or pets in the home. LPA has confirmed that the home has been approved by the Contra Costa County Fire Protection District on 09/02/2026, stating that the "fire clearance granted. Second floor and garage are off-limits to children".

The applicant’s health and safety training has been completed and First Aid/CPR certificate is current and expires on 06/02/27. A copy of the mortgage statement was obtained and shows control of property. The applicant has provided proof of the required immunization's, and the required mandated reporter training was completed on 02/02/26 and expires on 02/02/28. Safe Sleep practices, and Effects of Lead Exposure and testing requirements information brochures were discussed and provided. Applicant was reminded that children are never to be left in a parked vehicle when transporting.

Incidental Medical Services (IMS) policy was discussed. For IMS information, see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The applicant was reminded that ALL assistants, volunteers, frequent visitors, or adults living in the home, that are 18 years of age or older must be fingerprint cleared and associated to this facility prior to being in the presence of children in care or an immediate civil penalty will be assessed from $100 to $3000 per person, per incident. The applicant was reminded of the responsibility as a mandated reporter.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for and removing recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

On this date, 06/18/2026, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

The applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The applicant was reminded to conduct fire, earthquake, and disaster drills every 6 months.

The following was discussed with the applicant:

Capacity requirements, Roster requirements, Posting requirements, Documentation requirements for disaster drills (fire and earthquake). Mandatory Forms for the children’s files and provider’s files, and Safe Sleep Awareness. The role and responsibilities of being a mandated reporter were reviewed. The applicant was reminded that supervision is always required to children in care.

Applicant was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Applicant was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must always have the facility’s phone number; if the phone number is changed, licensing must be notified.

Regulation prohibits the smoking of tobacco in a private residence that is licensed as a family childcare home and in those areas of the family day care home where children are present (24/7 ban). State law prohibits baby walkers, bouncy seats, exercise saucers and any other items that fall into that category.

A copy of Safe Sleep Regulations was provided to the applicant and reviewed. LPA provided a copy of LIC 9227 and reviewed sleeping log with the applicant.

Applicant was encouraged to frequently visit our website at www.ccld.ca.gov for licensing regulations and updates.

As of 09/17/2026, LPA recommend this home to be Licensed as a Large Family Childcare Home.
