On 08/15/2024 at 10:00AM, Licensing Program Analyst (LPA) T. Syess-Gibson conducted an unannounced 1-Year Required inspection. LPA was greeted at the door by Kavitha Paramod who contacted the Administrator via telephone and explained the purpose of visit. Ellen Russell, Administrator arrived at 10:56AM and currently holds a certificate (#6024401740) that expires on 02/01/2025. The facility’s fire clearance was approved for six (6) non-ambulatory residents.

LPA toured the facility with Administrator including but not limited to bedrooms, bathrooms, kitchen, common area and back yard. The facility consists of five (5) bedrooms and three (3) bathrooms. All outdoor and indoor passageways are kept free of obstruction. LPA did not observe any bodies of water. A comfortable temperature is maintained at 75 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 106.2 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars. There is a minimum of 7-day supply of non-perishable and 2-day of perishable foods.

Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 03/10/2023. Emergency Disaster Plan was last posted on 08/01/2024. First aid kit was observed to be complete. Fire drill was last conducted on 12/11/2023.

Four (4) staff records were reviewed. During record review two (2) staff (S2) and (S3) were not associated to facility and (S3) did not have First Aid. LPA reviewed all four (4) resident records, and they were current and complete.

LPA requested updated copies of the following documents to be submitted to CCLD by 08/22/2024.

· LIC 308 Designation of Administrative Responsibility
· LIC 309 Administrative Organization
· LIC 500 Personnel Report
· LIC 610E Emergency Disaster Plan
· Liability Insurance

LPA observed the following deficiencies:

· At 10:43am, LPA observed Lysol disinfectant spray in the common bathroom's unlocked cabinet
· At 10:46am, LPA observed there were no non skid mats in all of the common bathrooms
· At 10:52am, LPA observed an unlocked small refrigerator with insulin inside the laundry room which
has no lock on the door
· At 11:00am, LPA observed the staff room on the facility sketch has been changed to a resident room without a fire clearance
· At 11:10am, LPA observed an unlock cabinet located in the kitchen with Clorox Bleach and knives
· At 12:55pm, LPA observed during record review two (2) staff (S2) and (S3) was not associated to the facility

*The total amount of civil penalties assessed on today's date is $1,000.00 for staff not being associated and $500.00 for Fire Clearance *
