On 8/4/2022 at 11:50 AM, Licensing Program Analyst (LPA) L. Francisco arrived unannounced to conduct Infection Control Inspection. LPA met with Facility Manager, Jude Yrreverra and explained the purpose of the visit.

During the Infection Control Inspection, LPA toured facility with Facility Manager including but not limited to front entrance, screening station, hand washing stations, bedrooms, common areas, and kitchen Facility has a sufficient 2-day perishable and one week non-perishable food supply. There is one central entry point for universal screening for staff, residents and visitors. A sign-in policy, thermometer and hand sanitizer were observed at screening station. Cough/sneeze etiquette, social distancing and hand washing posters were observed. Common touched surfaces are disinfected at least once daily. Bathrooms are equipped with liquid soap, paper towel and trash bin with touchless lids. Facility has a 30-day supply of PPEs maintained at central location and easily accessible for staff.

At 12:50 PM, LPA reviewed 4 staff records and 2 of 4 have health screening and TB test on file Facility has a mitigation plan and maintains record of routine screening for residents and staff.

THE FOLLOWING DEFICIENCY WAS OBSERVED
  • At 12:55 PM, LPA observed S1 and S2 does not have health screening and/or TB test on file

Personnel Requirements - General
(f) All personnel, including the licensee and administrator, shall be in good health, and physically and mentally capable of performing assigned tasks. Good physical health shall be verified by a health screening, including a chest x-ray or an intradermal test, performed by a physician not more than six (6) months prior to or seven (7) days after employment or licensure. A report shall be made of each screening, signed by the examining physician. The report shall indicate whether the person is physically qualified to perform the duties to be assigned, and whether he/she has any health condition that would create a hazard to him/herself, other staff members or residents. A signed statement shall be obtained from each volunteer affirming that he/she is in good health. Personnel with evidence of physical illness or emotional instability that poses a significant threat to the well-being of residents shall be relieved of their duties.

Based on record review, the licensee did not comply with the section cited above. LPA observed S1 and S2 does not have health screening and/or TB test results on file which poses a potential health and safety risk to persons in care.
