On 08/19/22 at 4:15 PM, Licensing Program Analyst (LPA) J. Sampair conducted a Plan of Correction inspection. Upon entry into facility, LPA explained the purpose of the visit with Licensee Estrellita Cruz..

The LPA conducted this POC visit as a check up on this facility where the staff were not associated with the facility when he last visited the facility. The LPA found that all caregivers present were associated. He also cleared all of the deficiencies from his previous inspection.

No deficiencies cited during this visit. Exit interview conducted and a copy of this report provided via email.
