On 8/20/26 at 1:10pm, Licensing Program Manager (LPM) Wynn Norona and Licensing Program Analyst (LPA) Briana Plumboy met with Applicant/Owner/Director Anuradha Sala for the purpose of a pre-licensing/ change in location inspection. Also present for the inspection was S1. Applicants requested an infant component for 6 infants ages 0- 18 months, a toddler component for 32 toddlers ages 18 months - 36 months, a preschool component for 30 preschool children ages 3 to 6 years old, and a school age component for 15 school age children ages 6-14 years old, for a total capacity of 63. The facility will operate Monday through Friday from 7:00am until 6:30pm. The fire clearance was granted on 7/6/2026. The facility will provide morning and afternoon snack as well as lunch. Applicant/Owner/Director is aware at no time shall the children in care (Infants, Toddlers, Preschool, and School Age children) commingle with another component.

LPA reviewed with Applicant/Owner/Director the LIC 311A, Records To Be Maintained At The Facility for child’s records, personnel records, administrative records, and documents to be posted.

LPA discussed the safe sleep regulations with Owner/ Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Senior Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

INDOOR ACTIVITY SPACE:
There is 1 infant classroom, 1 toddler classroom, 1 preschool age classroom, and 1 school age classroom. LPA observed a sufficient amount of equipment, toys, tables, chairs, cubbies, napping cots, and cribs. There is a first aid kit in each of the classrooms. Medications will be stored in each individual classroom, as well as in the refrigerator in the kitchen. LPA observed cleaning disinfectants are appropriately stored and inaccessible to children. Applicant/Owner/Director stated there are no poisons or firearms on the premises. LPA observed water pitchers in each classroom and Applicant/Owner/Director is aware water must be readily available to children in care at all times. LPA observed a functional carbon monoxide detector in each classroom. LPA observed a paper sign-in/sign-out system.

LPA measured infant classroom 1. The total classroom space contains 462.82 total of square feet which will accommodate applicant's request for 6 infants. LPA measured Toddler classroom 1 . The total classroom space contains 1386.77 total of square feet which will accommodate applicants request for 32 Toddlers. LPA measured Preschool classroom 1. The total classroom space contains 1120.42 total of square feet which will accommodate applicants request for 30 Preschool Age children. LPA measured School Age classroom 1. The total classroom space contains 978.50 total of square feet which will accommodate applicants request for 15 school age children. There are 10 toilets and 8 sinks for the children. There is a separate restroom for the staff located in the preschool restroom. Individual measurements are recorded on the Capacity Worksheet (LIC 9024). Children who become ill during the day will be isolated in the office area and will use the staff restroom, if necessary.

OUTDOOR ACTIVITY SPACE:
There are 2 outdoor play areas on the property, the playground located on same level of the childcare classrooms will be utilized by the infants and toddlers, and the preschool and school age children will utilize the playground located on the ground level of the center which can be accessed down a ramp in the back of the center. The play structure in the infant/toddler yard may only be utilized by 6-23 months, and the play structure located on the ground level in the preschool/ school age yard may only be utilized by 2-5 years old per manufacture guidelines. At no time shall children utilize a play structure which is not age appropriate. Once a toddler turns 24 months in age, they must utilize the preschool/ school age playground. Applicant must submit an outdoor schedule for the components. The outdoor play area is fenced with a medal fence that is at least four feet tall.

There are no bodies of water on the premises during today's inspection. There is a shaded area supplied by a canopy on the yard.

LPA measured the outdoor activity space. There are 2 outdoor plays areas. LPA measured the infant/toddler outdoor activity space which contains 990.17 square feet total. LPA measured the preschool/ school age outdoor activity space which contains 3307.09 square feet total. Individual measurements are recorded on the Capacity Worksheet (LIC 9024).


This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. LPA discussed the following: 100% supervision is required at all times, including in the bathroom; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPA discussed with Applicant/Owner/Director any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.

CURRENT FIRE CLEARANCE COMMENTS:

There are no comments on the approved fire clearance



Applicant/Owner/Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Owner/ Director was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication.

The facility is pending licensure for the following:


1) Outdoor schedules with times each component will play outdoors must be submitted for all components. On schedule indicate which playground will be used and how many children will utilized each play space. The request must contain the designated scheduled times for the infant, toddler, preschool, and school age children use for the outdoor play space.

2) Indoor activity space for infants shall be physically separate from space used by children in the child care. The center may use moveable walls or partitions to separate the above groups in the same room provided. Moveable walls or partitions, if used, shall be at least four feet high; shall be constructed of sound-absorbing material; and shall be designed to minimize the risk of injury to infants. A printout of Title 22, Division 12
Chapter 1, Subchapter 02, Section 101438.3 was provided to Owner/Director/ Applicant

3) A plan must be submitted for keeping the preschool age children separate from the school age children while using the restroom

4) Updated LIC 200A if Applicant/Owner/Director decides to change ages served


5) Updated fire clearance if Applicant/Owner/Director decides to remove ages served; school age program and increasing the preschool capacity from 30 to 45
