On 11/15/2024 at 1:30pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced annual required inspection. LPA met with Regina Lim, Administrator and explained the purpose of the visit. The administrator currently holds a certificate (#6006493735) that expires on 11/18/2024. The facility has a fire clearance for six (6) non-ambulatory clients.

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of five (5) total bedrooms and two (2) bathrooms. Two (2) bedrooms are occupied by staff. Pool in back yard is covered. A comfortable temperature for clients is maintained at 69 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 119.0 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for residents. There is a minimum of 7-day non-perishables and 2-day perishables foods.

Smoke detectors/carbon monoxide were in operating condition during visit. Fire extinguisher was last services on 11/8/2024. First aid kit was observed to be complete.

LPA reviewed all six (6) client and both staff record, and all are current and complete.

LPA observed the following deficiencies:
  • At 1:20pm, LPA observed refrigerator locked.
  • At 1:50pm, LPA observed facility was not conducting fire drills, and the last one was 11/26/2023.
  • At 12:05pm, LPA observed a T.V. stand, chair, stove, wheelchair, boxes of telephone wires, 2 microwaves and bags of pillows in back yard.

LPA requested the following documents to be submitted to CCLD by 11/22/2024.
  • LIC 308 Designation of Administrative Responsibility
  • LIC 500 Personnel Report
  • LIC 610D Emergency Disaster Plan (9 pages)
  • Updated facility sketch


Deficiencies are cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
