On 2/29/2024 at 2:50pm, Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct an annual year inspection. LPA met with House Manager, Yolanda Vengco and explained the purpose of the visit. Administrator was out-of-the-country. The facility's fire clearance is for two (2) ambulatory and four (4) non-ambulatory clients.

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of four (4) total bedrooms and two (2 ) bathrooms. One (1) bedroom occupied by staff. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 70 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 109.8 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for residents. There is a minimum of 7-day non-perishables and 2-day perishables foods.

Smoke detectors/carbon monoxide were in operating condition during visit. Fire extinguisher was last services on 10/23/2023. Emergency disaster plan last updated 2/3/2023. Fire drill last conducted 2/8/2024. First aid kit was observed to be complete.

LPA observed the following deficiencies:
  • At 4:00pm, LPA observed during record review four (4) clients age 60 or over.
  • At 4:00pm, LPA observed during record review none of the clients have an appraisal needs and services plan.


The deficiency were observed (see LIC809D) and cited from the California Code of Regulation, Failure to submit proof of correction (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.

Exit interview conducted. A copy of the appeal rights and this report provided.

85068.4 Acceptance and Retention Limitations

(g) If acceptance or retention of an individual 60 years of age or older would result in the number of persons 60 years of age or older exceeding 50 percent of the census in facilities with a capacity of six or fewer clients, or 25 percent of the census in facilities with a capacity over six, the licensee must request an exception in order to accept or retain the individual. The exception request must be made in accordance with Section 80024. The documentation specified in Section 85068.4(c) must be submitted with the exception request.
