On 01/19/2024 at 9:45AM, Licensing Program Analysts (LPAs) Carol Fowler and Tonica Syess-Gibson conducted an unannounced 1-Year Required inspection. LPAs met with Ramon Azucena, Caregiver, and explained the purpose of the visit. Christina Limjoco, Caregiver arrived at 10:21am. The Administrator Maria Honrada arrived at 11:28am. Administrator currently holds a certificate (#6005643735) that expired on 11/06/2023. The facility’s fire clearance was approved for six (6) ambulatory residents.

LPA toured the facility with Caregiver including but not limited to bedrooms, bathrooms, kitchen, common area, garage and backyard. The facility consists of five (5) total bedrooms which one (1) bedroom is occupied by staff, and three (3) bathrooms. No bodies of water observed. A comfortable temperature is maintained at 68 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 115.1 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars.

Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 03/31/2023. Emergency Disaster Plan was last posted on 8/1/2023. First aid kit was observed to be complete.

Reviewed Administrator and three(3) staff files and Five(5) client files. All five (5) client files were complete.

LPAs observed the following deficiencies:

· At 10:36am, LPAs observed a non-operating dishwasher.
· At 10:41am, LPAs observed Lysol, Clorox disinfectant wipes and Raid in a unlocked cabinet underneath kitchen sink.
· At 10:49am, LPAs observed spray paint, Lysol, Roach Motel and Pine sol in an unlocked shed in the unlocked garage along with a Ladder standing against the wall.

LPAs requested the following documents to be submitted to CCLD by 02/02/2024.

LIC 500 Personnel Report
LIC 308 Designation of Administrative Responsibility
LIC 309 Administrative Organization
LIC 400 Affidavit Regarding Client/Resident Cash Resources
LIC 402 Surety Bond
LIC 610E Emergency Disaster Plan


The deficiencies were observed (see LIC809D) and cited from the California Code of Regulation, Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
