On 1/17/2025 at 9:50am, Licensing Program Analysts (LPA) L. Hall, conducted an unannounced 1-year required inspection. LPA met with Marco Sullca Capcha, Direct Support Professional. Carol Duenas, Licensee arrived at 10:50am and LPA explained the purpose of the visit. Administrator certificate #7015855735 expires 6/11/2026. The facility’s fire clearance was approved for six (6) ambulatory clients.

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of four (4) total bedrooms and two (2 ) bathrooms. All indoor passageways are kept free of obstruction. Facility has a hot tub in front yard. A comfortable temperature for clients is maintained at 68 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 110.8 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Hand washing poster, paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for residents. There is a minimum of 7-day non-perishables and 2-day perishables foods.

Smoke detectors/carbon monoxide were in operating condition during visit. Emergency disaster plan last updated on 1/13/2025. Fire extinguisher was last serviced on 8/14/2024. Fire drill last conducted 11/14/2024. First aid kit was observed to be complete.

Eight (8) staff records were reviewed all were current and complete. All four (4) client records were reviewed and complete. LPA reviewed P & I.

LPA observed the following deficiencies:
  • At 9:50am, LPA observed a covered hot tub in front yard not gated.
  • At 10:30am, LPA observed during record review cash safeguard records were not accurate.


Deficiencies are cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.

Exit interview conducted. A copy of the appeal rights and this report provided.
