On 5/26/2023 at 12:10PM, Licensing Program Analysts (LPA) L. Hall arrived unannounced to conduct the annual 1-year required inspection. LPA met with Agatha Warance, Direct Support Staff (DSP) , and explained the purpose of the visit. LPA toured the facility with DSP, Agatha Warance. The administrator currently holds a certificate (#6041546735) that expires on 7/22/2024. The facility’s fire clearance was approved for six (6) ambulatory clients.

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of four (4) total bedrooms and two (2 bathrooms). All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 75 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 102.9 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition.

Smoke detectors/carbon monoxide were in operating condition during visit. Emergency Disaster Plan was last posted on 05/31/2022. Fire drill was last conducted on 5/22/2023. Fire extinguisher last services 11/14/2022. First aid kit was observed to be complete.

There were two (2) staff files available and both were incomplete. All three (3) clients records reviewed and all were incomplete. LPA was not able to review P & I only record (LIC405) for one (1) client. Staff stated for clients P & I is on an ATM card.

The following forms to be updated and submitted to CCLD by 06/02/2023:
  • LIC 500 Personnel Report
  • LIC 400 Affidavit Regarding Client/Resident Cash Resources
  • LIC 402 Surety Bond
  • Client's roster
  • LIC 308 Designation of facility's Responsibility
  • LIC 610D Emergency disaster plan (9 pages)

LPA observed the following deficiencies:
  • At 12:15PM, LPA two (2) clients left in facility alone. No staff present.
  • At 12:20PM, LPA was informed Administrator is still out-of-the country. (Will be cited on today's POC visit).
  • At 12:40PM, LPA observed facility does not have a land-line phone.
  • At 12:40PM, LPA observed hole in living room wall.
  • At 12:50PM, LPA observed insects underneath kitchen sink in cabinet, and insect traps on counter top in kitchen and cabinets.
  • At 12:53PM, LPA observed facility did not have a 7-day supply of non-perishables or 2-day supply of perishable foods available for clients.
  • At 12:55PM, LPA observed middle cabinet in bathroom vanity broken.
  • At 01:00PM, LPA observed a hole in R2's bedroom.

  • At 1:05PM, LPA observed a tire and a garden tool in the back yard on left side of house. And the entry gate to the back yard not able to close.
  • At 1:15PM, LPA observed during record review clients files incomplete. (Will be cited on today's POC visit).
  • At 1:20PM, LPA observed during record review only two (2) staff files and both were incomplete. (Will be cited on today's POC visit).
  • At 1:25PM, LPA observed during record review that facility does not have cash on site for P & I, and only has record for one (1) client.
  • At 1:45PM, LPA observed during record review one of C1's medications were not given.
  • At 2:05PM, LPA observed during record review one of C2's medications not matching MAR.

The following deficiencies are cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
