25 | On 1/31/2025 at 2:25pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 1/16/2025. LPA met with Nicolas Dedicatoria, Lead, and explained the purpose of the visit.
The incident occurred on 1/15/2025. Incident report stated C1 was upset about a package that was being delivered. C1 went outside and hopped on his ATV, rode in the back yard, rode in the front parking area, then sped passed staff down the hill. S1 stated during interview that S2 ran to try to catch him, while S1 retrieved keys for the vehicle. Staff lost sight of C1. S1 called Antioch Police Department. S2 found C1 approximately two minuets away from the facility. The Antioch Police arrived and placed C1 on a hold. C1 returned back to the facility the following morning.
Deficiency is cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
