13 | On 4/19/2022 at 1:45PM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to deliver complaint findings for the allegation above. Upon arrival, LPA met with Executive Director, Nofoatoo Afoa and explained to her the reason for the visit.
During the course of the investigation, the Department conducted interviews with staff, clients, witnesses, and complainant. Client’s physician’s report, care plan, care notes, medication list, medical records, and incident reports were obtained and reviewed.
Staff stated that C1 was receiving bed baths and would be wiped down as needed. C1 would refuse to shower and staff could not force C1 to shower. However, facility have not sought assistance from physician or behaviorist regarding C1’s hygiene needs. Witness stated that C1 smelt like urine and appeared to be in urine for hours or overnight based on the urine odor. (Continue on LIC9099C...) |
