25 | On 03/27/2024 at 9:30AM, Licensing Program Analyst (LPA) A. Gomez arrived unannounced to continue the 1-Year Annual Required inspection. LPA met with Executive Director, Caroline Frangieh and explained the purpose of the visit. The facility’s fire clearance was approved for 162.
Smoke detectors are interconnected with sprinklers and observed throughout the facility. Fire extinguisher was last serviced on 5/23/2023. Fire Drill was last conducted on 12/22/2023. Emergency Disaster Plan was last posted on 2/28/2024.
At 11:20AM, LPA reviewed 8 residents records. At 3:20PM, LPA reviewed 5 staff records.
THE FOLLOWING DEFICIENCIES WERE OBSERVED DURING VISIT:
- At 11:45 AM during file review LPA observed R1's physician report designating them as bedridden. R1 resides in room 194 which is not cleared for bedridden (Immediate $500 civil penalty) 87202(a)(2)
- At 3:20 PM during staff file review LPA observed that S2 and S3 do not have first aid training's and requirements on file.
- At 3:25 PM during staff file review LPA observed that S2 and S4 do not have a heath screen or TB result on file
***Immediate $500 Civil Penalty Assessed for Fire Clearance Violation***
The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiencies by POC date may result in additional Civil Penalties.
