On 06/27/2022 at 10:00 AM, Licensing Program Analyst (LPA) C. Lin arrived announced to conduct Pre-licensing Required inspection. LPA met with Executive Director Daniel Hogue and Director of Residential and Day Services Joseph Cashel, and explained the purpose of the visit. The facility is a brand new building and currently has no client.

LPA toured facility including but not limited to classrooms, conference rooms, staff offices, bathrooms, kitchen, common areas, and outdoor areas. Each room was equipped with the proper furniture. Bathrooms were equipped with shower and Handi Move ceiling mounted lift machine. Cabinets for medications and cleaning supplies were observed with locks. There is sufficient lighting throughout facility. Room temperature was maintained at 72 degrees F and hot water temperature was maintained at 109.2 degrees F. First-aid kit was observed to be complete and located in different rooms. Smoke detectors and carbon monoxide were operational. Fire extinguisher was last serviced on 01/28/2022.

No issues noted during inspection. LPAs observed that facility is ready to be licensed. This report will be submitted to the Central Applications Unit (CAU) and a final review of the application will be conducted. This facility is not yet licensed, and is subject to final approval by CAU. Additional requirements may still be required.

This Pre-licensing inspection is for facility's changing of location. Both Directors have been overseen Las Trampas multiple facilities for many years and are familiar with California regulations Title 22. Therefore, Comp III has been approved to be waived by LPM J. Fong.

Exit interview conducted with Directors and a copy of this report provided.
