On 9/10/2024 at 1:25pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced annual required inspection. LPA met with Angella Bernardino, Direct Support Professional, and explained the purpose of the visit. Administrator, Mike Nubla, arrived at 02:23pm. The administrator currently holds a certificate (#7033789735) that expires on 11/17/2025. The facility’s fire clearance was approved for five (5) non-ambulatory and 1 bedridden client.

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of five (5) bedrooms and two (2) bathrooms. One (1) bedroom occupied by staff and one (1) room used as an office. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 73 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 93.9 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Hand washing poster, paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for residents.

Smoke detectors/carbon monoxide were in operating condition during visit. Fire extinguisher was last services on 10/31/2023. Emergency disaster plan last updated on 2/1/2024. First aid kit was observed to be complete.

Three (3) staff files were reviewed and incomplete. Two (2) staff files were not available for review during visit. LPA reviewed 3 client files, one (1) of the clients was anew client. LPA reviewed P&I.

The following deficiencies were observed during visit:
  • At 3:00pm, LPA observed during record review that three (3) staff files were incomplete and all two (2) were not available for review.
  • At 3:30pm, LPA observed during record review facility was not conducting fire drill.


The following deficiencies are cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
