On 1/7/2025 at 3:35pm, Licensing Program Analyst (LPA) L. Hall, conducted an unannounced annual required inspection. LPA met with Enibokun Oyewusi, Administrator LPA explained the purpose of the visit. The facility’s fire clearance was approved for six (6) ambulatory clients.

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of three (3) bedrooms and one (1) bathroom. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 69 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 133.5 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Hand washing poster, paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for residents.

Smoke detectors/carbon monoxide were in operating condition during visit. The emergency disaster plan was last updated 6/19/2024. Fire extinguisher was last purchased on 12/5//2024. Fire drill last conducted 12/7/2024. First aid kit was observed to be complete.

LPA observed the following deficiencies:
  • At 4:35pm, LPA observed facility did not maintain a 7-day supply of non-perishables and 2-day perishable foods on the premises.
  • At 4:42pm, LPA observed facility did not maintain a quantity of linen to permit changing the linen at least once each week or more often.


The deficiencies are cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
