13 | Licensing Program Analyst (LPA) Soleil Marx, met with Director, Shannon Roberts, for the purpose of conducting an unannounced complaint investigation inspection alleging that children are fed food which they are allergic to. Throughout the course of the investigation, LPA made observations, conducted interviews, and reviewed records. It was determined by record review of child's file, posted allergy list, and staff interviews, that a child in care with a documented nut allergy was served peanut butter. The peanut butter was removed immediately by staff when they realized their mistake, however the child had already taken a bite of the restricted food. The facility notified authorized representatives immediately. The child did not have an allergic reaction nor did the child need medical attention, however the facility failed to adhere to the child's dietary restrictions. Based on LPAs record review and interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. A title 22 deficiency was cited on the subsequent page of this report. Exit interview conducted,report reviewed with Director, appeal rights provided. Notice of site visit given and must remain posted for 30 days.
