LPA Muscan conducted a case management on 5/22/24 to verify facility is closed. LPA Muscan toured the facility with Eskaton VP of Operations, Tighe Hammam to confirm closure. Facility walk through revealed there are no residents in care. Last resident moved out on December 26, 2023. Residents and their families were notified of the closure, as well as CCLD.

Currently the building is leased to Cal Fire for firefighters.

No deficiencies cited during today’s inspection.

No further action is needed. Exit interview conducted.
