25 | On 06/16/2026, Licensing Program Analyst (LPA) Aurelio Mendoza conducted an unannounced case management – incident inspection and met with Site Supervisor Jessica Zavala. LPA explained the purpose of the visit, which was to follow up on the unusual incident that occurred on 05/12/2026. LPA toured the indoor and outdoor areas of the facility accompanied by Site Supervisor Zavala.
According to the Unusual Incident Report submitted the same day, a preschool child fell while running on the sponge-foam playground surface at approximately 10:10 AM, landing on their left elbow. Staff responded immediately, comforted the child, and contacted the parent, who arrived at 10:39 AM and later sought medical care. The child received a cast and returned to care on 05/18/2026. Staff confirmed the cast has since been removed and the child is doing well.
During today’s inspection, the playground surface and layout were observed to be in good condition and free of hazards. Record review and staff interviews confirmed that 13 children and two qualified teachers were present at the time of the incident, and appropriate supervision was maintained. Staff reported reinforcing “walking feet” to promote safe play. Based on today’s observations, interviews, and records reviewed, the incident appears to be isolated, and staff responded appropriately by providing first aid, maintaining ratios, and promptly notifying the authorized representative. The LIC 624 was submitted within the required seven days.
Per Title 22, Division 12, Chapter 1, no deficiencies were cited. An exit interview was conducted with Site Supervisor Zavala. A copy of this report, the LIC 9058 (Appeal Rights), and the LIC 9213 (Notice of Site Visit) were provided. The LIC 9213 must remain posted for 30 days.
