Licensing Program Analyst (LPA) Lady Cabrera conducted an unannounced case management visit to the facility.

The purpose of the case management visit is to respond an incident report that was submitted to CCL Office 06/4/2021 and the incident occurred on 05/05/2021.

LPA met with Administrator Shaun Rushforth and stated the purpose of the visit. LPA interviewed Administrator regarding Staff (S1) witnessing potential abuse by S2 onto Resident (R1). Administrator stated an internal investigation was conducted. Administrator will obtain documentation regarding the investigation and will submit by Tuesday, 9/21/2021. LPA will follow-up with Administrator.
