Licensing Program Analyst (LPA) Lady Cabrera conducted an unannounced case management visit to the facility.

The purpose of the case management visit is to respond an incident report that was submitted to CCL Office 07/09/2021 and the incident occurred on 06/22/2021-06/24/2021.

LPA met with Administrator Shaun Rushforth and stated the purpose of the visit. LPA interviewed Administrator regarding R1 and to obtain additional information. Administrator reported R1 remains in the Independent Living Neighborhood.

No deficiencies observed.
