25 | On 12/18/2024 Licensing Program Analyst (LPA) Xona Xayavong conducted a Case Management-Incident inspection. LPA met with Director Aletria Snowden. LPA explained the reason for the inspection was to follow up on an unusual incident report. LPA Xayavong toured the facility, and a census was taken.
On 12/16/2024, the Fresno Regional Office received an unusual incident report stating that on 12/13/2024 S2 escorted C1 and C2 to the restroom. After returning back to the classroom, C2 asked S2 where C1 was and then S2 realized that C1 was left behind at the restroom.
During today’s inspection, LPA Xayavong conducted record review and interview with staff. Based upon staff interview, LPA determine there was lack of supervision as staff did not realize there was a child missing until C2 brought it to staff attention.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, a Type A deficiency is being cited on the attached LIC 809D.
Upon receipt of a Type A violation, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. A copy of the Fact Sheet - Child Care Parent Notification Requirements and a copy of LIC 9224 Acknowledgement of Receipt of Licensing Reports was given to Director Aletria Snowden. Per Director Aletria Snowden a completed signed copy of the LIC 9224 will be placed in each child's file.
An exit interview conducted with Director Aletria Snowden. A copy of this report and Appeal Rights were provided and discussed with Director Aletria Snowden. A Notice of Site Visit Form was posted to parent's board and must remain posted for 30 days. |
