On August 23, 2022, Fresno Child Care Regional office met with licensee, First Church of God of Fresno. Attendees were Regional Manager (RM) Rebecca Varela, Licensing Program Managers (LPM) Michael Duarte and Juvenal Moctezuma and Licensing Program Analyst (LPA) Cynthia Brannon. Representatives of licensee attended were Director Lisa Antaramian, Board Chair Rick Ransom, Board/Secretary Matt Phanco, Vice Chair School Board Joyce Hale, Chair School Board Pam Wirth, and Senior Greg Kendall.

The purpose of today’s office meeting is to review expectations of the licensee and application process. When a pre-licensing inspection is scheduled with licensee, it is with the understanding that the licensee has the intended licensed areas to be Title 22 compliance. Upon receipt of the application, there should not be any changes made by the applicant. This includes capacity changes, rooms to be licensed for a specific program and fire clearance is granted.

The application received on or there about of 11/25/19. The COVID epidemic did not stop the Regional office from conducting pre-licensing inspections. It is the Department’s goal to complete licensure within 90 days. The delays include a fire clearance denial dated 11/30/21, another fire clearance stating that not all rooms were ready for a fire clearance. A fire clearance was granted on 7/19/22 for 26 infants, 51 preschool children and 24 school age children. On 8/9/22, the final prelicensing inspection was conducted. Licensure was granted if licensee would correct items

During the preschool inspection, the following was observed:


1) The kitchen is accessible to children. The door leading into the kitchen is required to have a locking mechanism to ensure children do not have access to knives, sharp objects, etc. that are potentially harmful to children.

2) The changing table did not have the changing pad in place. Per director, there was a changing pad but was moved into another room. Licensee is to ensure that all changing pads are in place and if needed, purchase more to meet children's needs.
3) Some of the florescent light covers are missing or broken. Florescent light must have protective covers to provide a strong barrier to prevent light bulbs from shattering and the glass falling onto preschool children in care.
4) There are cleaning supplies in an unlocked cabinet. Cleaning supplies state to 'keep out of reach of children.' Items such as these, delay licensure. Facility must be set up as if day care services will be provided the next day.
5) During today's inspection, LPA found thumbtacks and hand sanitizers in unlocked drawers.
These items are a potential risk to children in care.

During the infant inspection the following was observed:
1) In infant classroom 8, there is an electrical outlet box that is not enclosed, thereby allowing infants to stick their fingers into the electrical outlet box and potentially receive an electrical charge/shock which is harmful to infants/children and adults.
2) There is a tall white cabinet with items on top, including a cardboard box that is hanging over the edge. The tall white cabinet is to be anchored and remove items hanging over the edge or items that may fall on top of infants.

During the school age inspection, the following was observed:
1) There were no cubbies for the 24 school age children. Per director, each classroom has bookshelves. They were supposed to be empty but licensee, First Church of God Early Enrichment Center, placed books and other items in areas that were designated for children's cubbies. Licensee and Director did not have bins set out for the requested capacity of 24 school age children. Licensee is required to provide space/cubbies for school age capacity.
2) In classroom 10, has window shades lying on the counter. This cannot occur when providing day care services to children.

3) There several vertical blinds that are torn in classroom 10. This item should not be present when providing day care services.
4) The fluorescence light covers are not fitting correctly or broken. Per director, the covers are outdated and the manufacturer does not produce them anymore. Per director, she can take the covers from other rooms that are not licensed and replace the broken covers.
5) The air vents in classrooms 10 and 11 are dirty. Vents should not be dirty when proving day care services to children. Licensee is to ensure that air vents are clean to help assist in air quality control and contain air contaminants.
6) In classroom 11, there are areas missing paint. One such area is under the small windows next to the classroom door. Per 101238, Building and Grounds, the childcare center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.
7) In classroom 11, the white board has a broken ledge and is not bolted to the wall. This white board can fall and hit a child in care.
8) There are cleaning supplies in the cabinet. Licensee has Betco, a disinfectant that states "Keep out of Reach of Children." During an inspection of a licensed facility, this accessible cleaning solution will be cited. A Type A citation can be issued.
9) Carpet needs to be cleaned in classroom 10. Children in care should not have to be in a room with dirty carpets. LPM observed dust emanating from carpet as he treads heavily through the classroom. Dust is known to trigger asthma attacks in children with respiratory issues.
10) Electrical outlet covers were not in place. Outlet covers help ensure that children do not place items into the outlets and injure themselves.
12) In classroom 11 has exposed wires wrapped around an electrical outlet that is protruding and not flushed against the wall. This is safety hazard to children in care.
13) Licensee has water pitchers with disposable cup dispensers in place in each classroom. However, in classroom 11, there are no disposable cups. Per director, the cups are in her office. When inspections are conducted, licensee is to set up the classroom as if day care service will be provided the next day.

14) The school age classrooms have student desks set up in each classroom. The desks and chairs are, in some cases, extremely dusty. Licensee did not clean the desks and chairs to ensure that children in care will have a clean desk and seat.
15) There is a wrought iron gate near the school age classrooms. This gate leads to Saginaw Way and to First Street, which is a busy street. To ensure children do not escape facility and place themselves in a potentially dangerous situation an alarm must be installed on the gate.
16) Staff are not to use the school age restroom. Upon arriving at facility, LPA/LPM observed that staff was using the school age children restroom. Whereas the school age program has not been licensed, staff must be aware and adhere to using the staff restroom.
17) The climbing structure is missing the age appropriate sticker.
18) There is not enough cushioning in the fall zones of the climbing structure.
19) Shade structure's lights is missing its cover.
20) Some of the tables in the school age play yard are broken.
21) Due to sharing the school age play yard, a waiver is required with play schedule to ensure commingling does not take place.

Management has decided to issue an increase to preschool capacity to 51 preschool children, with the understanding that all items listed above is corrected by Friday, August 12, 2022. A return unannounced inspection will be conducted. If above items are not corrected, citation(s) can be issued with possible civil penalties.

Moving forward, any changes requested by licensee, shall have an expectation to have facility meet Title 22 requirements and should not take more than 90 days.
