32 | Site Supervisor stated that the center will no longer accept volunteers until a screening procedure is put into place. Site supervisor stated that procedure will include the following: How volunteers will be screened and placed in classrooms that meets children’s needs. What type of training's volunteers will obtain in order to understand the importance of Licensing regulations such as, Children’s Personal Rights. Site Supervisor stated that she will host a training for all staff regarding procedures that ensures the safety of children in care. Even though staff did follow procedures, a child’s personal rights was violated while child was being cared and supervised by center staff.
LPA interviewed Site Supervisor, Day Care Staff, reviewed facility records, and obtained copies of reports of respected authorities. Investigation revealed through evidence obtained that the allegation, Facility staff hit daycare child, is SUBSTANTIATED. Based upon observations, and information gathered through interviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, this deficiency is being cited on the attached LIC 9099D. A Type A deficiency was cited on today’s date.
Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months." The licensee was provided a copy of the "Fact Sheet" for AB 633 (Parent Notification Requirements), along with a copy of the relevant documents this date. A completed signed copy of the LIC 9224 will be placed in each child's file.

An exit interview was conducted with Site Supervisor, Jacqueline Burrell. A copy of this report and Appeal Rights were provided and discussed with Site Supervisor, Jacqueline Burrell.
A Notice of Site Visit Form was posted to parent's board and must remain posted for 30 days. |
