25 | On 04/23/2024 Licensing Program Analyst (LPA) Stephanie Vega-Gonzalez, conducted an unannounced case management inspection that was initiated by licensee. LPA was met with Supervisor, Jacqueline Burrell and explained the reason for the inspection. LPA conducted a tour inside and outside and took a census of 43 children.
The department was notified on 04/18/2024 that the playgrounds tile's repairs had been completed. On today's date LPA toured the playground and observed the following: Tiles are in good working order. On the back of the outdoor shed there is a loose chain-link fence piece, "picture taken". LPA observed that the grass had scattered mushrooms throughout the yard, "pictures taken". LPA discussed findings with supervisor. LPA informed supervisor that it would be best practice to do a walk through of the yard before children are present, and to pick up any dangerous items including the mushrooms. LPA observed that on one of the play structure's a ladder and a slide had been made inaccessible with tape and a sign that states, "CLOSED" "picture taken." Supervisor informed LPA that preschool children are not allowed to use the large ladder and slide, and that staff put up tape to section area off everyday. LPA observed that the poles for the permanent shade had temporary cushioning. LPA was informed that the licensee will contact the department once the center has a permanent cushioning placed. As LPA walked through the yard under the tree and shade, LPA was bitten by mosquitos multiple times. LPA discussed findings with supervisor. LPA was informed that the school does spray yards once supervisor places a work order for mosquitos. Supervisor stated she would put in a work order and inform staff to keep children away from the shady area were the mosquitos are located.
As of today, 04/23/2024 the center's playground, playground tile, and permanent shade area are accessible to children in care. Once mushrooms have been picked.
