On 11/3/22, Licensing Program Analyst (LPA) Joseph Pacheco conducted an unannounced case management inspection. LPA met with Teacher, Yolanda Centeno and toured the facility indoors and outdoors. The purpose of the inspection was to follow up on the capacity decrease case management inspection conducted on 10/5/22 to verify that corrections were made.

During today's inspection LPA observed the following items have been corrected:

Inside Daycare:
· Install latch to lock kitchen door and make inaccessible to children.
· Provide documentation of a fire inspection for the newly requested capacity of 25.
· Submit an updated application (LIC 200A) to reflect the newly requested capacity of 25.
· Submit updated facility sketch (LIC 999) for indoor and outdoor spaces.
· Submit updated Personnel Report (LIC 500)
· Director, Foua Yang submitted to LPA a signed and dated letter on company letterhead from the owner of one relocation site. Facility is in process of obtaining a signed and dated letter from a second relocation site per Title 22 regulations.
· Remove yellow and black storage bins from the children’s bathroom
· Fix broken toilet.
· An updated Board Resolution Letter indicating requested changes Licensee has made.

Play Area:
· Fix gate that leads to Hansel & Gretel #2 located at 1327 N. Fresno St.

Pending a final file review, a recommendation will be made to license the above facility for the requested capacity of 25 infant children.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.

Exit interview conducted and report was reviewed with Teacher, Yolanda Centeno.

This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
