On 1/16/2024, Licensing Program Analyst (LPA) Stephanie Vega-Gonzalez conducted an unannounced Case Management inspection at facility. LPA met with Site Supervisor, Latrina Barbin who accompanied LPA during tour of facility both inside and outside. LPA explained the reason for the inspection. LPA obtained copies of facility records and interviewed staff.

On today’s inspection LPA inspected four classrooms. Two of the classrooms consisted of Toddler 1 with a census of 8 children and 2 staff. Toddler 2 class had a census of 5 children and 1 staff. The other two classrooms consisted of Preschool 1 with a census of 13 children and 2 staff. Preschool 2 had a census of 10 children with 2 staff. A total of 36 children were present at facility on today’s date.

Community Care Licensing (CCL) was notified of two incidents that occurred on 1/10/2024 and 1/11/2024. CCL was notified that on 1/10/2024 Child 1 was left alone, unsupervised for a total of five minutes inside of preschool classroom 1. LPA was informed that children had transitioned from indoors to outdoor play, when staff had conducted a headcount of children and noted that Child 1 was missing. Staff 1 quickly retrieved child from preschool classroom and brought child out to outdoor play area. CCL was also notified that on 1/11/2024 Child 2 walked out of preschool classroom 1 without the knowledge of any staff. It was confirmed via interview that Child 2 was wondering the front yard / walkway of the facility for a few minutes before staff 1 noticed Child 2 was wondering the walkway alone. Staff 1 took child back to preschool classroom 1.

On today’s date LPA observed that all children were individually sleeping in their own sleeping cots with lullaby music playing in the background. LPA observed that the front yard is under construction and is fenced by a chain link metal fence. LPA observed that the front yard from Toddler 2’s classroom, had various stacked materials on pallets. Materials are held together in a white wrap and is accessible to children in care if not fully supervised. LPA observed two adult construction workers working on the front play yard/ playground area. Site Supervisor, Latrina Barbin confirmed that construction workers had been working non-stop and had been present at facility during incidents.

LPA was informed that the following actions had been taken. Site Supervisor stated that there had been a staff meeting on supervision. Site Supervisor stated that staff are now scanning room to ensure that they had all children accounted for. Site Supervisor stated that preschool classroom 1 had a doorbell installed to ensure that no child is walking out of the classroom. LPA observed that doorbell was installed and functioning during inspection. Site Supervisor stated that staff will have an extensive training on supervision.

Per California Code of Regulations, Title 22, Division 12, Chapter 1, a Type A deficiency is being cited on the attached LIC 9099D.

Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months." The licensee was provided a copy of the "Fact Sheet" for AB 633 (Parent Notification Requirements), along with a copy of the relevant documents this date. A completed signed copy of the LIC 9224 will be placed in each child's file.

A Notice of Site Visit Form was posted to parent's board and must remain posted for 30 days.

Facility had submitted to CCL two UIR's incidents for 1/10/24 and 1/11/24 regarding Child 1 and Child 2 not being supervised. LPA interviewed Staff and it was confirmed that incidents did occur. This poses an immediate Health and Safety risk to children in care.
