25 | On 12/13/24, Licensing Program Analyst Miguel Herrera met with Site Supervisor, Latrina Barbin for an unannounced case management inspection. LPA toured the facility and took a census.
An Unusual Incident Report was submitted to the Fresno Community Licensing Office regarding an incident that occurred on 11/18/2024, where a preschool child was left alone in the classroom for an undisclosed amount of time while the classroom was in recess.
During today’s visit LPA Herrera interviewed staff that were involved in the incident. LPA Herrera also obtained and reviewed facility records pertinent to the incident. During the facility tour LPA conducted facility observations including the indoor and outdoor areas. Based on interviews it was determined that child #1 was left unsupervised for approximately 4 to 5 minutes.
LPA spoke to the Site Supervisor Barbin and reminded the facility to have visual supervision of children at all times whether they are outside or inside the classroom. The Site Supervisor indicated that she immediately reached out to all staff and provided supervision training and reminded all staff of the importance of child supervision, keeping count of children at all times, and communicating with other teachers present when taking children outdoors and indoors. The Site Supervisor stated that the facility conducted a Supervision Training on 12/02/2024. Although there was no immediate threat to child #1, it did pose a potential health, safety or personal rights risk to persons in care.
Exit interview conducted and report was reviewed with Site Supervisor, Latrina Barbin. Appeal rights were provided. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, one deficiency was cited during todays inspection (see LIC 809-D). This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
