25 | On 06/02/23, Licensing Program Analyst (LPA), Martha De Haro conducted an unannounced case management inspection to follow up on an Unusual Incident that occurred on 05/26/23. LPA met with teacher, Jose Valadez as well as Health and Safety Supervisor Charlotte Miranda. LPA toured the facility and a census was taken.
LPA De Haro informed Mr. Valadez and Ms. Miranda the reason for the visit. LPA interviewed Mr. Valadez and staff regarding the incident. The incident involved a child being left alone in the classroom while the rest of the class went out to the playground. Per Mr. Valadez, it was a case of miscommunication between staff as there were three extra people in the class, two of them from Early Learning. The staff from Early Learning were the last to stay in the classroom and did not make sure to check if a child was still in the class before they joined the rest of the class outside in the playground. Mr. Valadez immediately called the mother after the incident as the child was upset and crying.
Based on the information obtained, the facility staff did not take appropriate measures to assure that no children were left unsupervised inside of the classroom, which poses an immediate health, safety, or personal rights risk to children in care.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency was cited (See LIC809-D for additional information).
Exit interview conducted with Health and Safety Supervisor Charlotte Miranda. This report is to be made available to the public upon request. LIC 9213 Notice of Site Visit to be posted for 30 days. Appeal rights were also provided.
