On 2/27/2024, Licensing Program Analyst (LPA) Valentin Hernandez conducted an unannounced Case Management-Incident visit. During today’s visit, LPA met with Center Director, Tamikio Simpson, Teacher, Tammy Brummett and Teacher, Mandy Finkiewicz.

The reason for the Case Management visit was to follow up on an incident that occurred on 1/27/2025 and the LIC 624 form that was submitted due to that incident. The LIC 624 form documented that on Monday, January 27, 2025, at approximately 12:48pm the child got her right hand stuck in between the upper and lower doors of the children’s restroom that caused an injury to her middle index finger. Staff rinsed child’s hand under water, used a paper tower to apply pressure and gave an ice pack to the child.

Center staff called child’s parents and child was later picked up and taken to the hospital. Child had a small fracture in her right middle finger and was instructed to wear a finger splint.

During today’s Case Management visit LPA interviewed Center staff and inspected the restroom and door where the child hurt her finger. Through interviews it was determined that the child had supervision and that this incident was an isolated one. As a result of that incident the children were instructed not to put hands in the door and the center staff also received training.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiency has been cited during today’s visit. Center Director Tamikio Simpson was provided a copy of appeal rights.



A notice of site visit (LIC 9213) was given and must remain posted for 30 days. This report shall be made available to the public upon request.
