25 | Licensing Program Analyst (LPA) Aurelio Mendoza conducted an unannounced case management – other inspection at this Child Care Center and met with Director Rebecka Ramirez. This visit was consultative and intended to provide technical assistance regarding reporting requirements for unusual incidents under Title 22, Division 12, Chapter 1.
LPA reviewed Section 101212(d)(1)–(2) with the Director and clarified that unusual incidents that may affect the health, safety, or personal rights of children must be reported to the Department. These include, but are not limited to, injuries requiring medical treatment, suspected abuse or neglect, communicable disease outbreaks, emergency response incidents, poisonings, fires, or any occurrence that threatens a child’s well-being.
LPA reminded the Director that reportable incidents must be reported to the Department by telephone or fax no later than the next working day by contacting the Child Care Licensing Fresno Regional Office – Officer of the Day at (559) 243-4588. A written report (LIC 624) must then be submitted within seven (7) days with complete details of the incident, including what occurred, when and where it occurred, who was involved, parent notification, supervision at the time, and actions taken.
As a best practice, LPA directed the Director to complete the LIC 624 prior to contacting the Officer of the Day. Preparing the form in advance ensures staff are ready to provide accurate and complete information and helps determine whether the incident meets the criteria for a reportable unusual incident.
LPA also reminded the Director that parents or authorized representatives must be notified as soon as possible, but no later than the same business day, when an injury or unusual incident involves their child.
A current copy of the LIC 624 and reporting guidance was provided and reviewed. Technical assistance was offered to support continued compliance.
Per Title 22, Division 12, Chapter 1, no deficiencies were cited. An exit interview was conducted with Director Ramirez. A copy of this report, LIC 9058 (Appeal Rights), and LIC 9213 (Notice of Site Visit) were provided and discussed. The Director was advised that the LIC 9213 must remain posted for 30 days.
