13 | On September 24, 2021 Licensing Program Analyst (LPA) Kari McWilliams conducted an unannounced complaint inspection to discuss the allegation and provide findings regarding the above allegation. LPA met with Administrator Valerie Quiroz. LPA discussed the findings and took a census. This allegation was previously self-reported and investigated. During the course of the investigation, LPA interviewed director, and staff, reviewed facility records, and inspected facility.
On May 12, 2021 LPA conducted a Case Management Inspection. While transitioning indoors, Child #1 remained in the outdoor play area unsupervised. As a result of lack of supervision Child #1 was left in the outdoor play area unsupervised. The preponderance of the evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.
Per Title 22, Division 12, Chapter 1, a type A deficiency was cited on May 12, 2021 and a civil penalty was assessed. No additional citations are being issued on this date. Notice of Site Inspection to be posted for 30 days.
