25 | On 06/21/2022, Licensing Program Analyst (LPA) Luisa Gavoutian conducted an unannounced case management inspection. LPA met with Administrator Valerie Quiroz, toured the facility, and took a census. There were 35 children present at the facility. The purpose of today’s inspection was to review mandated reporter requirements for facility personnel.
During today’s inspection, LPA reviewed staff records and found that two staff had completed the mandated reporter “General Training,” but had not completed the “Child Care Provider’s training and one staff had not completed renewal mandated reporter training. Furthermore, all staff had a signed copy of form LIC 9108 – Statement Acknowledging Requirement to Report Child Abuse in their personnel records. LPA reviewed a sample of children’s records and found no documentation of suspected child abuse. LPA spoke to six facility staff, including teachers and Administrator, regarding the daily inspection of children, reporting of injuries or unusual incidents, and of the procedures for reporting suspected child abuse. Administrator stated she is aware of the reporting requirements for suspected child abuse and reporting such incidents to the Department. LPA reviewed these reporting requirements with Administrator.
California Code of Regulations (CCR) Section 101212(d)(1)(D) states, "(d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. (1) Events reported shall include the following: (D) Any suspected physical or psychological abuse of any child." (Continued LIC 809-C)
