On 07/07/23, Licensing Program Analyst Denisia Jimenez and Licensing Program Manager (LPM) Juvenal Moctezuma conducted an unannounced case management inspection to discuss an incident report that was submitted to Community Care Licensing regarding an incident that occurred at the facility on 06/20/23. LPA & LPM met with Beatriz Alvarez who stated they were in charge for the day since the site supervisor Tasha was on vacation.

Beatriz stated that on the date of the incident, Child #1 was heard crying and staff #1 walked over to see why child was crying. Staff #1 checked child to see if they had any scratches or bites but did not observe anything. Staff #1 attempted to pick up child and have them stand up but staff #1 noticed that child was not able to stand and started crying even more. Child #1 was taken to the front office and parents were called. Child was seen by their doctor where it was confirmed that they had a nondisplaced spiral fracture of shaft of right femur. Interviews conducted indicated that during the day of the incident, the center was within ratio. Through records reviewed and interviews conducted, it was confirmed that Child #1 did not have any incidents with other children or staff. LPM spoke to Beatriz and reminded them to have visual supervision of children at all times whether they are outside or inside the classroom. Beatriz indicated that she conducted a supervision training for the entire center on Monday 7/3/2023 and reminded all staff of the importance of child supervision. LPM also spoke about having separate group transitions when going out to play instead of having the entire classroom go out at once. Beatriz indicated that the center used to do that and will speak to the center supervisor to help with supervision and teacher child ratio.

This appears to be an isolated incident and staff took appropriate measures to address the child’s injury, following appropriate policies, regulations, and reporting requirements.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.
