25 | On 12/05/2024 Licensing Program Analyst (LPA), Stephanie Vega-Gonzalez, conducted an unannounced Case Management Inspection for the infant license. LPA met with Director, Phylicia Dahyelle Banuelos and toured the facility indoors and outdoors and a census was taken. LPA explained the purpose of the unannounced inspection.
The Department was notified that an incident had occurred on 11/20/2024 regarding Child 1 and the outdoor fence/ bolt metal parts that were exposed.
On today's date LPA came to facility to inspect the backyard chain link metal fence located in the backyard and to get an update on child 1. LPA observed that soft grey gel had been placed over the exposed bottom, middle, and upper bolts/metal on the line posts and terminal posts. LPA touched soft grey gel and noticed that it was tacky and not solid. LPA was able to pull pieces off the soft grey gel. LPA reviewed finding with director, Phylicia. LPA informed director that it would be best practice to remove gel and either sand down exposed metal areas and bolts or to find a permanent solution.. Director, Phylicia stated she understood.
Exit interview conducted and report was reviewed with Director, Phylicia Dahyelle Banuelos.
Appeal rights were provided.
Per Title 22, Division 12, of the California Code of Regulations, no deficiencies are cited.
This report shall be made available to the public upon request.
LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
