25 | On 06/27/2022, Licensing Program Analysts (LPAs) Jeovanna Yanez and Stephanie Vega-Gonzalez arrived at the facility to conduct an unannounced case management inspection. This report is a continuation of the Annual Required Inspection conducted on 6/14/22. LPAs met with Administrator, Cordelia Dixon-Hackett, provided the entrance checklist, and a census was taken.
LPA reviewed the facilities fire drill log, children files, and staff files that were unavailable for review during the annual inspection. A review of the facility records show that this facility is conducting a fire drill and documenting the date and time at least once every six months as required. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative/relatives/others who can assume responsibility for the child and medical assessment. LPAs reviewed a sample of staff files and observed files were complete with documentation of meeting qualification requirement and current documentation of completed mandated reporter training.
LPA discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are being cited.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with facility representative, Cordelia Dixon-Hackett. This report shall be made available to the public upon request.
