At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. The name of the childcare center director or fully qualified teacher(s) designated to act in the director’s absence has been reported to the Department. All children are under supervision, including visual supervision, of a teacher at all times. Facility meets teacher-child ratios. Based on LPA observations and records review and Director interview, S7 is a qualified aide with 9 units and is missing 3 units in child/family/community. During the tour, S7 was addressed as a qualified teacher supervising 10 PS in room 114. LPA addressed the above with Director. Director immediately moved S7 to supervise napping children and a qualified teacher to supervise room 114 for the remainder of the day. LPA reviewed CCLD's requirements for a qualified teacher and qualified aide. Director stated she understood and will ensure compliance moving forward.
LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representatives and or relatives or others who can assume responsibility for the child and medical assessment.
LPA reviewed a sample of staff files and observed files were not complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training. Based on LPA records review and Director interview, S4 does NOT have Mandated Reporter Training (MRT) Certificate in file. In addition, S3, S5 and S7 completed ONLY the general training for MRT not AB1207. LPA addressed the above with Director. Director stated staff will complete training and submit copies of certificates to LPA for review.
Based on LPA records review and Director interview, S1, S3, S4, S5 and S8 are missing physician’s stamp on LIC503. LPA informed Director new staff or current staff wanting to renew their LIC503 must obtain a physician's stamp to be valid. Director stated she understood and will ensure compliance moving forward.
Lastly, based on LPA records review, Director and S2 interview, S2 does NOT have TB test with result in file. S2 stated she will complete the requested and submit copies to Director to be maintained in her file. Director stated she understood and will ensure compliance moving forward.
LPA discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to resources such as forms, regulations Provider Information Notices (PINs), and Quarterly Updates.
LPA discussed Reporting Requirements as outlined in the regulations (Section 101212).
